GENERAL INFORMATION

Cardiology Consultants of Houston, L.L.P. was established in 1980 and has grown to a five-man group, specializing in cardiovascular diseases.  All physicians in the group are on staff at The Texas Heart Institute and their practice is centered at The St. Luke’s Episcopal Hospital in the Texas Medical Center.  All physicians are also clinical members of the faculty of Baylor College of Medicine and University of Texas – Houston. 

Physicians of this practice are available 24-hours a day, seven days a week.  Office consultations are arranged, by appointment, Monday through Friday.   Our office practice is supported by experienced registered nurses and medical assistants and by an experienced nuclear medicine team. 

Our cardiologists are all Board Certified in Cardiovascular Diseases and Internal Medicine and are available for office and hospital consultations.  Areas of interest and expertise include management of heart and vascular disease including coronary disease, hypertension, and heart failure.  Physicians at Cardiology Consultants of Houston, LLP perform some of the most complex coronary and peripheral vascular interventions, including balloon angioplasties, atherectomy and placement of stents. Management of cardiac arrhythmias, placement of pacemakers, defibrillators and monitoring devices are services offered by our heart failure electrophysiologist. 

We serve as clinical investigators in the development of new forms of therapy for a variety of cardiovascular diseases. 

We welcome new patients into our practice.  Each new patient who consults with a physician at Cardiology Consultants of Houston, L.L.P. will have a complete evaluation, along with review of any past medical records, lab results, test results…etc.  If referred to our office by your primary care physician, or other specialist, every effort will be made to coordinate care with that physician. 

There are several forms we ask every new patient to read and fill out.  If you choose to fill out the forms in our office, we ask that you arrive 20 minutes prior to your scheduled appointment time to do this.  For your convenience forms may be downloaded from Cardiology Consultants of Houston’s Website.  There is a button located on the left-hand side of the page labeled Downloadable Forms.  Click on that button, then on New Patient Forms or Established Patient Forms.  Each section will list the necessary forms that will be needed to complete your chart.  These forms can be filled out in the convenience of your home and brought with you at the time of your visit or they may be faxed back to our office ahead of time.   

If you are a member of a managed care health insurance plan, please bring your authorization or referral form with you to your scheduled appointment.  If your primary care physician’s office is to forward this paperwork, please contact us prior to your appointment time to ensure this step has been completed. 

Any medical records you would like reviewed should be forwarded to your physician at Cardiology Consultants of Houston, L.L.P. prior to your appointment date.  You may request your other physician(s) to mail your records to our office.  Or, you may obtain a copy, for your own records, to bring with you by completing a Records Release Authorization form and presenting it to the physicians’ office that has your records.  Please complete and send this form as far in advance of your appointment as possible, and please allow several weeks to have this information copied and transferred.
